Primary Care Provider Peak Performance Actions
- Physician should provide an Asthma Action Plan for the student to provide to the school. The form available in this program can be used if needed.
- Physician should obtain the appropriate form for self carry and medication in the school such as the Peak Performance USA Administration of Medication Authorization Form for the student to provide to the school.
